New Student Registration Requirements
  
Evidence of Date of Birth
  1. Metro Birth Certificate
  2. State Birth Certificate
  3. Delayed Birth Certificate
  4. Other specified types:
    1. Hospital Certificate
    2. Military Certificate
    3. Passport
    All children must be:
    • Four years old on or before September 30th to attend Pre-K
    • Five years old on or before September 30th to attend Kindergarten
    *Tennessee birth certificates can be obtained at Lentz Public Health Department-Vital Records Office: 340-5611, 311 23rd Avenue North, Nashville, TN.

    Immunization Record (Required Tennessee Child Health Form)

    Physical Examination - Please see the back of this form for a list of clinics that will complete a physical for school enrollment.

    Proof of Residence (must be a current lease, mortgage or utility bill)

    THE PARENT/GUARDIAN MUST PROVIDE ALL THE PROPER INFORMATION BY THE DATE SCHOOL STARTS OR THEIR CHILD WILL NOT BE ADMITTED UNTIL REGISTRATION REQUIREMENTS ARE MET. All ELL students must register through Randalls Learning Center (298-8467).
