Free Immunization Clinics
Every month the Metro Public Health Department offers free immunizations at various locations. These services are by appointment only and are at no cost. Families with private insurance should visit their primary care provider.
School Health Forms
Health and medication permissions forms for students are on the Metro Public Health Department website.
The forms include the self-administration of medication to nurse-directed administration of medication.
Medical Resources
