2026-2027 Permission Forms: What to Expect

This form confirms that you have reviewed the 2026-2027 Student-Parent Handbook and understand the policies in it, including compulsory attendance requirements under Tennessee law.

You will be asked to:

• Acknowledge that you have read the handbook.
• Acknowledge the attendance and truancy policy, including how unexcused absences can affect out-of-zone placement.

If you do not respond: This is not an opt-out form. Signing the form does not change your student’s responsibility to follow school policies.

Federal law allows schools to share basic directory information, such as a student’s name, photo and dates of attendance, unless a parent or guardian opts out.

• You will be asked whether you give permission to share your student’s directory information or would like to withhold specific categories of information.
• If you do not respond: Permission to share directory information is assumed granted.

This form covers routine hearing, vision and blood pressure screenings offered at school.

• You will be asked a yes or no question about whether your student has permission to participate.
• If you do not respond: Permission is assumed granted.

This form explains the MNPS internet use policy and filtering practices.

• You will be asked a yes or no question about whether your student has permission to access the internet at school.
• If you do not respond: Permission is assumed granted.

This form covers whether your student can be issued a laptop or similar device for learning on and off campus.

• You will be asked a yes or no question about whether your student has permission to use a district device.
• If you do not respond: Permission is assumed granted.

This form covers two free, confidential support services available at every school: Group-based school psychologist services and school social work counseling

• You will be asked two separate yes or no questions, one for each service.
• If you do not respond: Permission is assumed granted for both services.

The Navigator Program connects students with a staff member who checks in with them throughout the year about academic progress and well-being. The program uses a journaling tool called Sown to Grow.

• You will be asked a yes or no question about whether your student has permission to participate.
• If you do not respond: Permission is assumed granted.

The Transitions Program supports students moving between school levels, such as Pre-K to elementary school or middle school to high school. Activities include goal-setting and college and career readiness.

• You will be asked a yes or no question about whether your student has permission to participate.
• If you do not respond: Permission is assumed granted.

The Universal Behavior Screener is part of the district’s Multi-Tiered System of Support framework. This nationally normed screener helps identify students who may need additional behavioral support.

• You will be asked a yes or no question about whether your student has permission to participate.
• If you do not respond: Permission is assumed granted.

MNPS may share limited student and parent or guardian information with approved community partners to connect families with mental health services, counseling and resources to meet basic needs.

• You will be asked a yes or no question about whether your student has permission to participate.
• If you do not respond: Permission is assumed granted.

This form covers three separate situations involving your student and media.

You will be asked three yes or no questions:

• Whether MNPS can publicly recognize your student and share that information with the media.
• Whether MNPS can interview, photograph or record your student for district use.
• Whether outside news media can interview, photograph or record your student.

If you do not respond: The first two permissions are assumed granted. The third, allowing access by outside news media, is assumed not granted unless you give permission.

This form covers periodic surveys about school climate, relationships and student well-being. It does not include curriculum or instructional surveys.

• You will be asked a yes or no question about whether your student has permission to participate. Your response will carry forward to future school years.
• If you do not respond: This form requires your permission. Without it, your student may not be able to participate in district surveys covered by this form

Only applies if a student's parent or legal guardian currently serves in the military. State and federal reporting requirements call for identifying these students to better understand outcomes for military-connected families.

You'll be asked to provide the serving parent or guardian's name, start date of service, service classification, and branch.

Part of the Lifetime Wellness course, covering topics like abstinence, healthy relationships, consent, reproductive systems, and related health and safety law topics.

• You'll be asked whether your student can participate in all lessons, or only in specific topics you select.
• If you don't respond: Permission is assumed granted.

Applies to students in grades 9 through 12.

This optional form supports federal reporting requirements tied to career and technical education programs.

You'll be asked four yes or no questions covering circumstances like single parenthood, workforce status, homelessness, and foster care history.

Federal law requires school districts to share contact information for high school students approaching military service eligibility age with military recruiters, unless a parent opts out.

• You'll be asked a yes or no on permission to share your student's contact information with a military recruiter, along with basic contact details.
• If you don't respond: Permission is assumed granted, and the information is shared.
