2026-2027 Permission Forms: What to Expect

Each year, Metro Nashville Public Schools asks parents and guardians to review and respond to a set of permission forms. These forms cover things like health screenings, internet and device use, school support services and media participation.

To meet web accessibility standards, we are no longer posting the full permission packet as a PDF on our website. Instead, this page explains what is included so you know what to expect before completing your forms online.

How These Forms Work

Most of these forms work on an opt-out basis. If you do not respond, the district assumes permission is granted and your student will participate.

A few forms work the opposite way and require your explicit “yes” before your student can participate. We have noted which is which below for each form.
